白藜芦醇是什么?
白藜芦醇(Resveratrol,RSV)是一种二苯乙烯类化合物(3,5,4′-三羟基-反式-二苯乙烯),属于天然的非黄酮类多酚类,分子式C14H12O3,分子量228.2431。RSV是一种脂溶性化合物,难溶于水,易溶于乙醇、乙醚等有机溶剂。作为一种植物抗毒素,白藜芦醇由某些植物在受压(如紫外线)、受损或受到病原体(如细菌或真菌)攻击时产生2,3。食物中白藜芦醇的来源包括葡萄皮、蓝莓、树莓、桑椹和花生等4。白藜芦醇以二种几何异构体的形式存在:顺式-(Z)和反式-(E),如图1所示,自然界中主要以反式结构存在。顺式和反式白藜芦醇与葡萄糖结合(白藜芦醇-3-O-β-葡萄糖苷,简称白藜芦醇糖苷)是其主要的衍生物5。当暴露于紫外线照射时,反式可以发生光异构化为顺式6,7。

第一次提到白藜芦醇是在1939年来自日本Michio Takaoka的一篇文章,次年他从百合科藜芦属植物白藜芦(Veratrum album)中分离出白藜芦醇,后来在1963年从日本虎杖(Polygonum cuspidatum)根中分离获取8,9。1976年,在葡萄中也发现了白藜芦醇。
代谢与生物利用度:
白藜芦醇在体内广泛代谢,肝脏和肠道是其代谢的主要部位,口服容易吸收,代谢后通过尿液及粪便排出10,11。葡萄糖醛酸化发生在肠道和肝脏中,而磺化不在肝脏和肠道中发生,并通过微生物肠道活动产生12。由于代谢快,白藜芦醇的半衰期很短(约8-14分钟),但硫酸盐和葡糖醛酸代谢产物的半衰期超过9小时11。对反式白藜芦醇在人体内的药代动力学进行的初步研究发现,口服单剂量5-25mg的反式白藜芦醇后,血浆中只有微量的未代谢的白藜芦醇。事实上,由于口服反式白藜芦醇容易吸收、快速代谢和消除,其生物利用度很低13,仅约0.5%14。一旦被吸收,白藜芦醇通过与葡萄糖醛酸和/或硫酸盐结合而快速代谢,形成白藜芦醇葡萄糖醛酸、硫酸盐和/或磺基葡萄糖醛酸。硫酸盐缀合物是在人类血浆和尿液中发现的白藜芦醇代谢产物的主要形式15。
初步研究发现,健康志愿者单次口服25mg反式白藜芦醇在约60分钟后,血液中总白藜芦醇(即反式白藜芦醇及其代谢物)的峰值浓度约为1.8-2μmol/l(μM),这取决于白藜芦醇是在葡萄酒、蔬菜汁还是葡萄汁中给药14,16。一项针对40名接受单次递增剂量口服反式白藜芦醇(即0.5g、1g、2.5g和5g)的健康受试者的研究表明,未代谢的白藜芦醇血浆浓度在白藜芦醇给药后0.8至1.5小时达到峰值,水平范围为0.3μM至2.3μM17。值得注意的是,这些数值明显低于体外实验中白藜芦醇的化学预防作用(>5μM)。相反,在单次口服5g反式白藜芦醇后,发现某些白藜芦醇缀合物的峰值血浆浓度比未代谢的白藜芦醇高约2至8倍17。此外,与单剂量给药相比,发现29天内每天重复摄入5g的反式白藜芦醇会导致反式白藜芦醇和两种白藜芦醇葡萄糖醛酸缀合物的峰值血浆浓度显著升高18。重复服用1g/天的反式白藜芦醇(该剂量不太可能引起副作用,见下文安全性)可产生约22μM的白藜芦醇-3-O-硫酸盐(人体中最丰富的硫酸盐缀合物)最大血浆浓度,约7-8μM的典型单葡糖醛酸缀合物最大血浆浓度19。
一些研究考察了食物基质对白藜芦醇吸收和/或生物利用度的影响20。一项研究报告称,红酒中反式白藜芦醇的生物利用度在用餐时(低脂肪或高脂肪)与空腹时没有差异21。然而,在另一项研究中,补充白藜芦醇的吸收被发现由于胃中存在食物而延迟,但没有减少22。第三项研究发现,补充白藜芦醇的生物利用度会因饮食中的脂肪量而降低,但不会因同时服用槲皮素(另一种多酚)或酒精的共同给药而降低23。
关于白藜芦醇在人体中的生物利用度的信息很重要,因为迄今为止进行的大多数实验研究都是“临床前”的,即在体外将细胞暴露在比在人体中观察到的峰值血浆浓度高出100倍的白藜芦醇浓度下,以及在给予非常高剂量(非膳食)白藜芦醇的动物模型中20。当消化道中的细胞暴露于未代谢的白藜芦醇时,其他组织可能暴露于白藜芦醇代谢产物。目前,人们对白藜芦醇代谢产物的生物活性知之甚少。然而,如果一些组织能够将白藜芦醇代谢产物转化回白藜芦醇,那么组织中稳定的白藜芦醇缀合物可以作为体内白藜芦醇再生的储备库13,19。
白藜芦醇的生物活性:
白藜芦醇的作用机制主要是改善氧化应激、减轻炎症反应、改善线粒体功能和调节细胞凋亡(自噬)和神经保护等延长许多模式动物的寿命。白藜芦醇可能是一种有效且安全的化合物,可能用于预防和治疗衰老和与年龄相关性的多种疾病24。3.1. 减少氧化应激:这是衰老过程以及年龄相关疾病发生和发展的关键因素。过量ROS诱导的脂质过氧化、蛋白质过氧化和防御系统受损逐渐损害细胞结构和功能,最终导致细胞衰老并加速衰老过程25。综合分析表明,白藜芦醇可以抑制氧化应激的形成,从而发挥抗衰老作用26。
在试管中,白藜芦醇有效地清除(中和)自由基和其他氧化剂27,28,并抑制低密度脂蛋白(LDL)氧化29,30。白藜芦醇可诱导抗氧化酶,包括超氧化物歧化酶(SOD)、硫氧还蛋白、谷胱甘肽过氧化物酶-1、血红素加氧酶-1和过氧化氢酶,和/或抑制烟酰胺腺嘌呤二核苷酸磷酸氧化酶(NOX)产生活性氧(ROS)31。然而,几乎没有证据表明白藜芦醇在体内是一种重要的抗氧化剂。口服后,人体内白藜芦醇的循环和细胞内水平可能远低于其他重要抗氧化剂,如维生素C、尿酸、维生素E和谷胱甘肽。此外,白藜芦醇代谢产物的抗氧化活性可能低于白藜芦醇32。
3.2. 抑制炎性反应:
慢性低度炎症水平通普遍存在于衰老过程中。越来越多的证据强调了炎症在衰老和与年龄相关的代谢疾病进展中的重要性,它是老年人发病率和死亡率的一个非常关键的风险因素33,34。研究表明,白藜芦醇通过改善炎症反应而发挥抗衰老作用35,36。
3.3. 改善线粒体功能:
已知线粒体功能障碍和线粒体含量下降是衰老的标志,在促进衰老中发挥着重要作用37,38。越来越多文献报道,白藜芦醇可调节线粒体功能,从而延缓衰老过程39,40。
3.4.调节细胞凋亡:
作为细胞内或细胞外损伤的结果,细胞凋亡被激活作为维持体内平衡的适应性反应。一方面,凋亡导致无功能和受损细胞的消失,如癌症细胞。另一方面,其失调在年龄相关疾病的发展中起着至关重要的作用。例如,过度的神经元凋亡促进了神经退行性疾病的发展41。许多研究表明,白藜芦醇可以调节细胞凋亡,防止衰老和与年龄相关的疾病42,43。
3.5.作为热量限制的模拟物
热量限制(CR),即在保持最佳营养的同时,将饮食摄入量降低到能量需求以下,是唯一已知的有可能延缓衰老的营养干预措施44。大量研究表明,热量限制(CR)是促进长寿和健康衰老的金标准45。白藜芦醇和CR在SIRT1信号传导方面表现出相似的抗衰老活性,而且10μM白藜芦醇在体外和高剂量组(100mg/kg体重)在体内表现出比CR更强的抗衰老和刺激SIRT1水平的活性46,总之提示了白藜芦醇作为CR模拟物(CRM)的潜力46,47。
3.6.雌激素和抗雌激素活性:
内源性雌激素是由人类和其他哺乳动物合成的类固醇激素。这些激素与细胞内的雌激素受体结合。雌激素受体复合物与DNA中的独特序列(雌激素反应部件;EREs)相互作用,以调节雌激素反应基因的表达48。
白藜芦醇与合成雌激素激动剂己烯雌酚(Diethylstilbestrol)的化学结构非常相似(如图2),这表明白藜芦醇也可能作为雌激素激动剂发挥作用,即可能与雌激素受体结合,并引发对内源性雌激素的类似反应。然而,在细胞培养实验中,发现白藜芦醇可作为雌激素激动剂或雌激素拮抗剂,这取决于细胞类型、雌激素受体亚型(ERα或ERβ)和内源性雌激素的存在等因素48。最近,在动脉损伤动物模型中,白藜芦醇通过ERα依赖性途径改善内皮伤口愈合49。

白藜芦醇影响寿命
2003年,一种名为白藜芦醇的植物化合物成为抗衰老医学的热门话题。哈佛大学的一项研究显示,给予白藜芦醇的酵母寿命惊人地延长了70%50。几项后续研究支持了补充白藜芦醇的模式生物的寿命益处51。正如上所述,白藜芦醇作用机制包括诱导了一些与热量限制(CR)相关的有利基因表达变化50。这导致白藜芦醇出现在数千篇已发表的关于其可能预防的多种疾病的论文中,包括200次以上的临床试验。2021年发表的一篇评论文章总结道:“白藜芦醇可能是一种有效且安全的化合物,用于预防和治疗衰老和与年龄相关的疾病。”52已发表的研究记录了白藜芦醇在实验室模型中延长寿命的能力。
- 一项对19篇已发表论文的荟萃分析表明,白藜芦醇是一种延长生命的药物53。研究的物种包括酵母、蛔虫、小鼠、果蝇和绿松石溪鱼。
- 白藜芦醇已被证明可在试管(体外)和蛔虫体内(体内)的人体细胞中诱导自噬52。自噬是一种清洁过程,可以促进内部细胞碎片的清除,促进细胞成分再循环回胞质溶胶。因此,自噬是调节细胞功能的基本机制,包括DNA修复、增殖、分化、胚胎发育和免疫反应54。白藜芦醇诱导自噬被认为是一种延长寿命的机制54。
- 喂食白藜芦醇糖浆的蜜蜂比对照组活得更长55。根据白藜芦醇浓度的不同,这些蜜蜂的平均寿命和最长寿命分别增加了33%至38%。
- 喂食不同浓度白藜芦醇的短命苍蝇的平均寿命延长了10%至29%,而其他模型发现白藜芦醇也具有神经保护作用51,56,57。
- 与对照组相比,喂食白藜芦醇的鱼类寿命更长,认知能力和运动功能更好58。
- 在一种易患神经退行性疾病并加速衰老的基因改变小鼠中,口服白藜芦醇可将这些小鼠的中位生存期从32天增加到42天59。白藜芦醇还有助于保护运动功能,防止大脑发生退行性变化。
不过,并不是所有的研究都证明了这种延长寿命的现象。一项研究发现,白藜芦醇延缓了大鼠的血管衰老,但对总生存率没有影响60。另一项研究发现,在喂食标准饮食的小鼠中,白藜芦醇并不能延长寿命61。然而,在喂食高热量饮食的小鼠身上,白藜芦素将死亡风险降低了31%,并提高了胰岛素敏感性,这表明它有助于预防与饮食相关的代谢疾病62。
尽管白藜芦醇增加了试管中Sir2同源人SIRT1的活性54,但没有流行病学数据将白藜芦醇、SIRT1激活和延长人类寿命联系起来。此外,增加人类SIRT1活性所需的白藜芦醇的超生理浓度显著高于口服后在人类血浆中测量的浓度。
白藜芦醇与神经退行性变
5.1.与神经退行性疾病相关的生物活性
大脑衰老受到神经炎症、自噬失调、线粒体功能障碍、细胞损失和氧化状态升高的折磨。这一切都会导致记忆丧失和运动障碍,最终会增加患神经退行性疾病的风险,如阿尔茨海默病(老年痴呆)、帕金森病和痴呆症63,64。大量数据显示白藜芦醇是如何预防啮齿类动物神经退行性疾病的52。补充白藜芦醇的动物表现出记忆力提高、神经递质分泌增加、新脑细胞产生增加,并有益地减少炎症和氧化应激,对神经退行性疾病表现出预防或保护作用65-68。
5.1.1.刺激神经发生和微血管形成
与年龄相关的情绪变化和记忆缺陷是由老年人海马体功能下降引起的。白藜芦醇可刺激健康老年大鼠海马的神经发生和血管形成。这些结构变化与空间学习、记忆形成和情绪功能的显著改善有关66。
5.1.2.刺激β-淀粉样肽清除
阿尔茨海默病(AD)的一个特征是β-淀粉样肽积聚到AD患者海马和皮层神经元外的老年斑块(淀粉样)中69。老年斑块对细胞有毒,导致进行性神经元功能障碍和死亡。白藜芦醇被发现有助于培养物和神经元细胞系中原代神经元的β-淀粉样肽的清除并促进细胞存活70-72。白藜芦醇还降低了转基因AD小鼠模型不同脑区的老年斑块计数73。
5.1.3.抑制神经炎症
AD大脑中老年斑块周围异常活化的小胶质细胞和肥大的星形胶质细胞释放细胞毒性分子,如促炎介质和ROS,增强β-淀粉样肽的形成和沉积,并进一步损伤神经元69。白藜芦醇被发现能够抑制小胶质细胞系和脑淀粉样蛋白沉积小鼠模型中由β-淀粉样肽诱导的小胶质细胞活化引发的炎症反应74。据报道,在用白藜芦醇治疗的健康老年大鼠中,小胶质细胞活化和星形胶质细胞肥大的发生率也有所降低66。
5.14..减少氧化应激
线粒体功能障碍和氧化应激被认为与几种神经退行性疾病的病因和/或进展有关75。白藜芦醇在培养的神经母细胞瘤中对抗氧化应激和β-淀粉样肽诱导的毒性76。白藜芦醇处理的原代神经元细胞对氧化应激相关损伤的抵抗力与血红素加氧酶-1(HO-1)的诱导有关,HO-1是一种降解促氧化血红素的酶77。在中风的实验模型中,白藜芦醇在野生型小鼠的缺血再灌注过程中限制了梗死面积,但在缺乏HO-1基因的小鼠中没有限制78。此外,白藜芦醇能够纠正实验诱导的大鼠氧化应激和相关的认知功能障碍79。
5.2.改善认知功能的临床研究
尽管白藜芦醇对大脑的生物利用度尚不确定80,但一项随机、双盲、安慰剂对照研究报告称,单次口服500mg白藜芦醇后,健康年轻受试者(18-29岁)前额叶皮层的脑血流量增加。然而,白藜芦醇的摄入并不能改善给药后进行的认知需求任务的表现81。
最近,在一项为期26周的双盲安慰剂对照研究中,对46名健康、超重的老年人(年龄50-80岁,BMI 25-30 kg/m2)联合给药白藜芦醇(200mg/天)和槲皮素(320mg/天),显著改善了记忆功能测试并提高血糖控制作用82。白藜芦醇可能模仿热量限制(CR)对认知健康的代谢益处的其他证据可能来自正在进行的健康老年人和AD患者的临床试验83。
在阿尔茨海默病(AD)的小鼠模型中,热量限制已被证明可以限制神经毒性β-淀粉样肽在大脑中的产生和沉积84。与热量限制的作用类似,发现白藜芦醇通过激活代谢传感器,包括SIRT和AMP活化蛋白激酶(AMPK),改善肥胖和糖尿病相关的代谢失调85,并促进AD小鼠模型大脑中β-淀粉样肽的AMPK依赖性清除72。如前所述,白藜芦醇在培养细胞和动物模型中也表现出额外的神经保护特性。
白藜芦醇与心血管疾病
6.1.与心血管疾病预防相关的生物活性6.1.1.抑制血管细胞粘附分子(VCAM)的表达
动脉粥样硬化是一种炎症过程,其中脂质沉积在动脉壁内的斑块(产生动脉粥样硬化)中,并增加心肌梗死的风险86。动脉粥样硬化发展中最早的事件之一是血管细胞粘附分子将炎症性白细胞从血液募集到动脉壁87。已经发现白藜芦醇抑制培养的内皮细胞中粘附分子的表达88,89。
6.1.2.抑制血管平滑肌细胞(VSMC)增殖
血管平滑肌细胞(VSMCs)的增殖在高血压、动脉粥样硬化和再狭窄(当治疗的动脉再次堵塞时)的进展中起着重要作用。已发现白藜芦醇在培养物90-92和体内93中抑制VSMCs的增殖。白藜芦醇似乎通过ERα依赖性机制减少VSMC增殖,从而防止动脉损伤小鼠模型中的血管狭窄93。
6.1.3.刺激内皮一氧化氮合酶(eNOS)活性
eNOS是一种催化血管内皮细胞形成一氧化氮(NO)的酶。NO是维持血管舒张所必需的,而NO依赖性血管舒张受损与心血管疾病风险增加有关94。由于发现生理浓度的白藜芦醇可刺激培养的内皮细胞中的eNOS活性95-97,白藜芦醇可能有助于在体内维持或改善内皮功能。
6.1.4.抑制血小板活化和聚集
血小板聚集是血栓形成的第一步,血栓会阻塞冠状动脉或脑动脉,分别导致心肌梗死或中风。已经发现白藜芦醇在体外抑制血小板活化和聚集98-100。
6.2.预防心血管疾病的证据
6.2.1.红酒多酚
适度饮用酒精饮料可显著降低心血管疾病风险101。“法国悖论”(French Paradox)—尽管法国的饮食饱和脂肪和吸烟水平很高,但其冠心病发病率相对较低—导致人们认为经常饮用红酒可能会对心血管疾病提供额外的保护102。红葡萄酒含有不同浓度且通常较低的白藜芦醇(见下文来源部分)和较高浓度的类黄酮,如原花青素。这些多酚化合物在试管和一些动脉粥样硬化动物模型中显示出抗氧化、抗炎和其他潜在的抗动脉粥样硬化作用103。不过,针对这一问题的流行病学研究结果并不一致。虽然一些大型前瞻性队列研究发现,葡萄酒饮用者患心血管疾病的风险低于啤酒或白酒饮用者104-106,但其他研究没有发现差异107-109。喜欢葡萄酒的人和喜欢啤酒或白酒的人之间的社会经济和生活方式差异可能解释了一些研究中观察到的部分额外好处:喜欢葡萄酒的人们往往比喜欢其他酒精饮料的人收入更高,受教育程度更好,吸烟更少,吃更多的水果和蔬菜,摄入的饱和脂肪更少109-114。
尽管适度饮酒一直与降低冠心病风险有关,但尚不清楚红酒多酚是否能进一步降低风险。有趣的是,对啮齿动物服用不含酒精的红酒的研究发现,与心血管疾病相关的各种参数都有所改善115,116。一项安慰剂对照的人类研究发现,服用红葡萄多酚提取物的心脏病患者的内皮功能有了急性改善117。除了与红酒酒精含量相关的益处外,增加红酒多酚的消费是否还能提供任何额外的心血管益处,还需要进一步研究118。
6.2.2.白藜芦醇与内皮功能
内皮功能障碍通常与心血管危险因素(如胰岛素抵抗、高血压和高胆固醇血症)的存在有关,并被认为先于心血管和代谢紊乱的临床表现。内皮功能障碍的特征是血管收缩异常、白细胞粘附于血管内皮细胞、血小板活化和聚集、平滑肌细胞增殖、血管炎症、血栓形成(凝块形成)、凝血障碍和动脉粥样硬化119。
实验研究:白藜芦醇已被发现在体外对心血管系统具有许多保护作用,包括抑制血小板活化和聚集98,120,121,通过增强NO产生促进血管舒张97,以及控制炎症脂质介质的产生122-124。然而,产生这些作用所需的白藜芦醇浓度通常高于口服白藜芦醇后在人体血浆中测得的浓度14。一些动物研究还表明,高剂量的白藜芦醇口服可以降低血栓形成和动脉粥样硬化的风险125,126,尽管一项研究发现,喂食白藜芦醇的动物会增加动脉粥样硬化127。白藜芦醇在体内的其他保护作用包括在各种模型中减少心肌肥大和降低血压,以及限制心肌梗死后大鼠的梗死面积20。
随机对照研究:在一项为期六个月的交叉研究中,34名代谢综合征患者被随机分配接受白藜芦醇(100mg/天)治疗三个月,要么在研究开始时立即接受,或者在三个月后接受。补充白藜芦醇可提高肱动脉血流介导扩张(FMD)的值,这是血管健康的替代标志。然而,停用白藜芦醇后三个月内,FMD恢复到基线值128。研究的局限性是,白藜芦醇制剂含有额外的化合物(即维生素D3、槲皮素和米糠植酸酶),这也可能影响内皮功能。一项针对健康超重或肥胖志愿者(BMI,25-34kg/m2)的随机安慰剂对照研究发现,单剂量反式白藜芦醇(30mg、90mg或270mg)在给药后60分钟左右改善了臂部FMD129。在第二项研究中,同样的研究人员发现,无论参与者在六个月内接受单剂量白藜芦醇(75mg)还是每日剂量(75mg/天白藜芦醇),FMD的改善都是相似的130。
在一些额外的研究中,白藜芦醇被证明可以通过减少血管炎症和内皮激活来改善内皮功能。一项针对41名健康受试者的随机、双盲、安慰剂对照研究发现,连续一个月每天补充白藜芦醇(400mg)、葡萄籽提取物(400 mg)和槲皮素(100mg)可显著降低内皮细胞中白细胞介素-8(IL-8)和细胞粘附分子(ICAM-1和VCAM-1)的表达,这表明其对内皮功能障碍具有保护作用131。在一项为期一年的随机、双盲、安慰剂对照研究中,对75名心血管疾病(CVD)高危人群进行了富含白藜芦醇的葡萄补充剂与不含白藜芦醇葡萄补充剂的每日摄入量的比较。给予富含白藜芦醇的补充剂(白藜芦醇:8mg/天,持续6个月,然后16mg/天再持续6个月)显著改善了循环炎症标志物的状况,降低了C反应蛋白(CRP)和肿瘤坏死因子-α(TNF-α)的水平,以及血栓形成因子、纤溶酶原激活物抑制剂-1(PAI-1)的水平132。六个月后,两种CVD风险标志物氧化低密度脂蛋白(oxLDL)和载脂蛋白B(ApoB)的浓度降低,进一步表明白藜芦醇具有心脏保护作用133。用相同的方案补充稳定型冠心病患者也改善了循环炎症标志物的分布,并降低了外周血单核细胞(PBMC)中促炎基因的表达134。在补充患者的PBMC中,还发现特异性参与动脉粥样硬化和促炎信号的微小RNA和细胞因子的表达下调135。最后,尽管尚不清楚高血压是内皮功能障碍的原因还是影响,但最近一项随机对照试验的荟萃分析表明,补充高剂量白藜芦醇(≥150mg/至少一个月)可能有助于降低心血管疾病风险个体的收缩压136。

白藜芦醇对癌症的作用
7.1.与癌症预防有关的生物活性
7.1.1.对生物转化酶的影响
一些化合物在体内被I期生物转化酶代谢后才具有致癌性,尤其是细胞色素P450酶138 。通过抑制某些细胞色素P450酶的表达和活性139,140,白藜芦醇可能通过限制致癌原的激活来帮助预防癌症。相反,增加II期解毒酶的活性通常会促进潜在毒性或致癌化学物质的排泄。研究已发现白藜芦醇可增加培养细胞中NAD(P)H:醌氧化还原酶-1(NQO1)的表达和活性141,并且可能是其他II期酶的弱诱导剂142。
7.1.2.抑制增殖和诱导细胞凋亡
DNA损伤后,如果损伤不可修复,则细胞周期可以被暂时阻断,以允许DNA修复或激活导致细胞死亡(凋亡)的途径143。细胞周期调节缺陷可能造成突变的传播,从而导致癌症的发展。此外,与正常细胞不同,癌细胞增殖迅速,并且无法对启动细胞凋亡的细胞死亡信号做出反应。白藜芦醇已被发现在许多癌细胞系中诱导细胞周期阻滞和/或凋亡(程序性细胞死亡)20。
7.1.3.抑制肿瘤侵袭和血管生成
癌细胞在基质金属蛋白酶的帮助下侵入正常组织。已经发现白藜芦醇抑制至少一种类型的基质金属蛋白酶的活性144,145。为了促进其快速生长,侵袭性肿瘤还必须通过一种称为血管生成的过程来发展新的血管。已经发现白藜芦醇在体外146-148和体内149抑制血管生成。
7.1.4.抗炎作用
炎症促进细胞增殖和血管生成,并抑制细胞凋亡150。白藜芦醇已被发现在体外抑制几种炎症酶的活性,包括环氧化酶和脂氧化酶151,152。白藜芦醇也可能抑制促炎转录因子,如NFκB或AP-1153,154。这意味着白藜芦醇可以阻断癌性肿瘤的生长,也可以阻碍已有癌症的生长和转移。
7.2.白藜芦醇防癌抗癌的研究
据报道,年龄是癌症发生和发展的最重要风险因素之一155。大量研究表明,白藜芦醇主要通过抑制细胞增殖、诱导细胞凋亡和抑制细胞迁移来有效预防肿瘤形成和治疗癌症。
白藜芦醇已被发现可抑制多种人类癌细胞系的增殖,包括乳腺癌、前列腺癌、胃癌、结肠癌、胰腺癌和甲状腺癌细胞系138。在动物模型中,白藜芦醇的摄入、局部应用和/或注射在许多部位抑制了化学诱导的癌症的发展,包括胃肠道、肝脏、皮肤、乳房、前列腺和肺部156,157。在啮齿类动物模型中,白藜芦醇的抗癌作用包括减少细胞增殖、诱导细胞凋亡以及抑制血管生成、肿瘤生长和转移20。然而,一些动物研究报告称,口服白藜芦醇在抑制香烟烟雾中致癌物诱导的肺癌发展方面缺乏作用158,159,白藜芦醇给药治疗结肠癌研究的结果不同160-162。
目前,尚不能完全确定白藜芦醇是否对人类癌症的预防和/或治疗有益。人体研究中报道的白藜芦醇的生物利用度较低,限制了白藜芦醇对人体可能的全身健康影响的临床评估(见上文代谢和生物利用度部分)。然而,在一项先导研究中,在20名癌症患者的结肠直肠肿瘤组织中检测到未代谢的白藜芦醇和缀合物,这些患者每天口服补充4g或8g白藜芦醇29天。白藜芦醇似乎具有良好的耐受性,与基线相比,白藜芦醇显著(尽管适度)减少了细胞增殖163。在一项小型随机、双盲、安慰剂对照试验中,向6名患有结直肠癌和肝转移的患者服用白藜芦醇微粒化制剂(名为SRT501)14天,旨在增加白藜芦醇对靶组织的递送164。在服用5g SRT501的六分之五的患者的肝脏中可以测量到未代谢的白藜芦醇,并且SRT501给药导致肝肿瘤组织中凋亡标记物裂解胱天蛋白酶-3(Cleaved caspase-3)的检测增加。然而,在一项针对多发性骨髓瘤患者的非随机、非盲试验中,SRT501的给药与包括肾衰竭在内的许多严重不良反应有关,因此试验暂停165。由于肾衰竭是骨髓瘤患者的常见并发症,目前尚不清楚肾衰竭病例是否应仅归因于SRT501的使用。然而,在探索白藜芦醇在临床环境中的假定益处之前,需要找到安全的方法来提高其在人体中的生物利用度13,166。
2017年的一项综述表明,白藜芦醇已被证明可以逆转癌症细胞的耐药性,使其更容易受到以前不够有效的癌症治疗;当与其他药物联合使用时,它可能能够提高化疗治疗的疗效167。
白藜芦醇辅助治疗糖尿病
8.1.糖耐量受损与2型糖尿病
全球糖耐量受损(或称糖尿病前期)的人群较高且不断增长,导致患2型糖尿病的风险增加168。已知糖耐量受损与骨骼肌的胰岛素抵抗(IR)有关,骨骼肌是胰岛素介导的葡萄糖摄取的主要外周组织,胰腺β细胞的胰岛素分泌也有缺陷。肌肉IR被认为是2型糖尿病发展的最早阶段,其特征是脂质暴露过多、胰岛素受体信号传导失调、葡萄糖摄取受损、线粒体功能障碍、脂肪酸氧化减少和促炎细胞因子表达增加。
在动物研究中,白藜芦醇已被证明可以改善肥胖和/或代谢异常动物的胰岛素敏感性、葡萄糖耐受性和脂质分布169。2011年对啮齿类动物进行的一项研究发现,白藜芦醇可降低高血糖动物的血糖水平,这是细胞内葡萄糖转运增加的结果。它也可能改善某些动物的胰岛素作用,或减少其他动物的胰岛素分泌170。
在人类中,短期补充白藜芦醇对2型糖尿病患者的葡萄糖和脂质代谢产生有益影响。在一项随机、双盲、安慰剂对照研究中,对70名2型糖尿病受试者进行了口服白藜芦醇补充剂(1000mg/天,持续45天)对糖代谢控制的影响评估171。安慰剂组和干预组的基线和研究结束测量之间的变化比较表明,白藜芦醇显著降低了空腹血糖和空腹胰岛素浓度,并改善了糖化血红蛋白(HbA1c)水平和胰岛素敏感性(HOMA-IR)的测量。此外,HDL胆固醇水平升高,而LDL胆固醇水平和收缩压显著降低。舒张压、总胆固醇、甘油三酯和肝功能标志物的测量没有变化171。另外,在一项随机、开放标签和对照研究中,评估了62名2型糖尿病患者口服白藜芦醇(250mg/天)对血糖控制和脂质代谢的影响172。在为期三个月的研究期间,与对照组相比,白藜芦醇组显著改善了生化和临床参数的变化,包括空腹血糖浓度、HbA1c水平、收缩压和舒张压、总胆固醇和LDL胆固醇。在一项为期四周的随机安慰剂对照研究中,低至10mg/天的白藜芦醇剂量也可降低19名2型糖尿病男性受试者的胰岛素抵抗173。
2017年发表的一项系统综述发现,有证据表明补充白藜芦醇有益于2型糖尿病。白藜芦醇有助于2型糖尿病患者的葡萄糖稳态和逆转胰岛素抵抗174。
8.2.白藜芦醇与肥胖:
肥胖(定义为体重指数BMI≥30kg/m2)是2型糖尿病发展的一个众所周知的风险因素。一些临床研究评估了白藜芦醇对超重或肥胖受试者关键代谢变量的影响,这些受试者没有明显的代谢功能障碍,并发现白藜芦醇治疗后几乎没有或根本没有代谢益处175-177。此外,目前没有可用的证据表明,糖耐量受损的超重或肥胖个体是否可以从白藜芦醇补充剂中受益,并降低他们患2型糖尿病的风险178。
目前的数据表明,白藜芦醇可以改善2型糖尿病患者的特定代谢变量179,180,但还需要更多的研究来评估其对糖尿病风险人群的影响,包括糖耐量受损的肥胖人群181。
白藜芦醇与其他年龄相关性疾病
9.1.对少肌症的影响
肌萎缩(Sarcopenia,肌肉减少症)是一种与年龄相关的衰老性综合征,其特征是肌肉质量和功能的逐渐丧失182,183。许多研究表明,白藜芦醇能有效地改善骨骼肌的质量和功能,延缓少肌症的发生,并且与运动相结合有更好的效果184-187。这种作用可能通过抑制细胞凋亡、促进线粒体生物发生和功能以及激活Sirt1来介导。
白藜芦醇有助于增加肌肉质量。2015年对鱼类进行的一项研究表明,白藜芦醇可以增加体重并诱导肌肉生长188。此外,它似乎可以减少鱼类肌肉的氧化应激,这意味着更快的恢复时间和更少的酸痛。
9.2.白藜芦醇与生殖功能
在雄性大鼠中,白藜芦醇增加了睾酮和精子数量,没有任何不良反应。它激活下丘脑-垂体-性腺轴(HPG轴),通过大脑中的垂体控制性激素从下丘脑释放。白藜芦醇通过平衡对雌激素的反应,影响男性和女性的生殖健康189。
9.3.白藜芦醇改善骨骼健康
白藜芦醇可以提高骨密度、强度和整体骨骼健康190。白藜芦醇激活脂肪燃烧途径(SIRT1),阻断增加脂肪储存的基因(PPAR-gamma)。这种表观遗传学转变导致干细胞发育成骨构建细胞,这可能会促进骨骼健康191。白藜芦醇还与关键的骨骼构建维生素如维生素D和维生素K2协同作用,保护骨骼并增加其矿化作用192。
白藜芦醇来源和稳定性
10.1.白藜芦醇来源作为一种植物抗毒素,白藜芦醇是许多植物在受到病原体感染或受到切割、挤压或紫外线辐射的物理伤害时产生的一种多酚化合物8。能够合成白藜芦醇的植物包括日本虎杖(Knotweeds,Reynoutria japonica)根、松树(包括苏格兰松和东方白松)、葡萄藤、树莓、桑椹、花生植物、可可灌木和越橘灌木,这些灌木可以产生浆果,包括蓝莓、蔓越莓和越橘4,8。
白藜芦醇在植物中通过白藜芦醇合成酶(Stilbene synthase,芪合成酶)产生,其直接前体是衍生自丙二酰辅酶A和4-香豆素酰辅酶A的四酮体193,194。后者来源于苯丙氨酸195。
10.2.食物来源
白藜芦醇存在于葡萄、葡萄酒、葡萄汁、花生、可可和越橘属浆果中,包括蓝莓、越橘和蔓越莓196-199。在葡萄中,白藜芦醇主要存在于葡萄皮中200。葡萄皮中白藜芦醇的含量因葡萄品种、地理来源和真菌感染情况而异201。
葡萄酒与葡萄皮接触的发酵时间也是其白藜芦醇含量的重要决定因素。因为在白葡萄酒和桃红葡萄酒的生产过程中,葡萄皮会在早期被去除,所以白葡萄酒通常比红葡萄酒含有更少的白藜芦醇202。因此,由于葡萄酒类型、年份、批次和地区之间的差异,很难准确估计全球酒厂数千种葡萄酒中的白藜芦醇含量203。然而,葡萄酒中的白藜芦醇含量通常很低,变化很大,不可预测,而白藜芦醇只是葡萄和葡萄酒多酚中的一种次要化合物20。
葡萄和葡萄汁中白藜芦醇的主要形式是反式-葡萄糖苷-3-β-葡萄糖苷,葡萄酒中含有大量的白藜芦醇苷元,这被认为是发酵过程中糖分解的结果5,196。许多葡萄酒还含有大量的反式白藜芦醇(见表1),这些物质可能在发酵过程中产生,也可能从葡萄苷(白藜芦醇聚合物)中释放出来204。红酒是白藜芦醇的一种相对丰富的来源,但红酒中存在的其他多酚的浓度远高于白藜芦醇205。

如表1所示,白藜芦醇在红葡萄酒的含量取决于具体葡萄品种。总的来说,由黑皮诺(Pinot noir)和圣卢朗(St. Laurent)品种的葡萄制成的葡萄酒显示出最高水平的反式白藜芦醇206。

花生,尤其是发芽的花生,正在发芽和发芽后的含量与葡萄相似,分别为2.3至4.5μg/g和11.7至25.7μg/g,具体取决于花生品种8,207。10.3.白藜芦醇稳定性
如前所述,白藜芦醇脂溶性、难溶于水中。粉末形式的反式白藜芦醇在75%湿度和40°C空气存在的“加速稳定性”条件下是稳定的208。反式异构体也通过转运蛋白的存在而稳定209。发酵后长期储存的葡萄皮和葡萄渣中白藜芦醇含量也很稳定。
安全性和副作用
11.1.剂量与不良反应白藜芦醇在美国是被认可的一种膳食补充剂。
只有少数人体研究确定了白藜芦醇的不良影响,所有这些都是初步的,参与者人数很少。不良反应主要由长期使用(数周或更长时间)和每日剂量1000mg或更高引起,导致恶心、胃痛、胀气和腹泻等210。在约1000mg的短期剂量下,它似乎没有不良副作用。
一项对七项研究中136名连续一个月服用500mg以上药物的患者的回顾显示,25例腹泻、8例腹痛、7例恶心和5例胀气211。2018年一项关于白藜芦醇对血压影响的综述发现,一些人排便和稀便的频率增加212。
在2500mg的高剂量下,白藜芦醇会导致非酒精性脂肪肝(NAFLD)患者恶心、呕吐、腹痛、腹泻和肝功能障碍213。一项评估10名受试者口服反式白藜芦醇安全性的试验发现,单剂量5000mg不会产生严重的不良反应17。根据2018年的二项综述文献,即使摄入白藜芦醇高达5000mg,也似乎是安全的214,215。
11.2.妊娠和哺乳期
含白藜芦醇补充剂在妊娠和哺乳期的安全性尚未确定216,不建议服用补充白藜芦醇。
11.3.雌激素敏感条件
在对白藜芦醇在人类中的雌激素活性有更多了解之前,有雌激素敏感癌症病史的女性,如乳腺癌、卵巢癌和子宫癌,应避免服用白藜芦醇补充剂(见上文:雌激素和抗雌激素活性)216。
11.4.药物相互作用
11.4.1.抗凝和抗血小板药物
白藜芦醇已被发现在体外抑制人类血小板聚集98,217。从理论上讲,当与华法林(香豆素)和肝素等抗凝药物一起服用时,高剂量摄入白藜芦醇(作为补充剂)会增加瘀伤和出血的风险;抗血小板药物如氯吡格雷(Plavix)和双嘧达莫(Persantine),以及非甾体抗炎药(NSAIDs)包括阿司匹林、布洛芬、双氯芬酸、萘普生等。
11.4.2.细胞色素P450代谢的药物
细胞色素P450(CYP)酶是I期生物转化酶,参与从内源性分子到治疗剂的广泛化合物的代谢。人类肝脏和肠道中最丰富的CYP异构体是细胞色素CYP3A4,它催化许多种药物的代谢218。据报道,白藜芦醇在体外219,220和健康志愿者142中抑制CYP3A4活性。因此,高剂量摄入白藜芦醇可能会降低CYP3A4进行广泛首过代谢的药物的代谢清除率,从而增加这些药物的生物利用度和毒性风险。
CYP3A4代谢的许多药物中的一些包括HMG-CoA还原酶抑制剂(他汀类药物)、钙通道拮抗剂(非洛地平、尼卡地平、硝苯地平、尼索地平、尼群地平、尼莫地平和维拉帕米)、抗心律失常药(胺碘酮)、HIV蛋白酶抑制剂(沙奎那韦)、免疫抑制剂(环孢菌素和他克莫司)、抗组胺药(特非那定)、苯二氮卓类药物(咪达唑仑和三唑仑)以及用于治疗勃起功能障碍的药物(西地那非)。
值得注意的是,最近完成的一项临床试验(NCT01173640)检测了单剂量和多剂量白藜芦醇(1000mg)干扰健康志愿者咪达唑仑代谢的可能性210。其他CYP酶(例如CYP2D6和CYP2C9)也可能被白藜芦醇抑制221。
最后,白藜芦醇被发现是CYP1A2表达和活性的弱诱导剂,CYP1A2催化几种药物的代谢,包括对乙酰氨基酚(扑热息痛)和抗抑郁药物克罗米帕明和丙咪嗪142,218。这表明白藜芦醇可能通过增加药物清除率来干扰CYP1A2介导的药物代谢,可能将循环药物浓度降低到治疗水平以下。
白藜芦醇用量
建议用量:白藜芦醇的适当剂量可能因其使用健康条件而异。作为一种膳食补充剂,典型的每日剂量范围为50-500mg,没有明确的最佳剂量。此外,在一些临床研究中,受试者被给予了更高的剂量。对于具体健康状况的用量,可参考上述有关的临床试验。白藜芦醇成分与产品
本文主要针对国际市场上白藜芦醇补充剂产品介绍其来源。13.1.天然来源:
白藜芦醇主要从虎杖根、葡萄(皮、叶)和红葡萄酒提取等获得。
- 国际市场上,大部分白藜芦醇原料从虎杖(Knotweed,又名日本虎杖)根中加工提取而得216,以获得标准化的白藜芦醇成分,例如反式白藜芦醇含量50%、75%和98%以上。
- 葡萄中白藜芦醇的含量很低,加工提取成本高。不过,葡萄因其食用历史悠久、生物活性更优且安全无毒等特点而闻名。因此,国际市场上以红酒、葡萄皮等为原料提取的白藜芦醇价格远高于虎杖提取物,二者原料价格存在巨大差异。
不过,在膳食补充剂的市场上,来源萄葡的标准化的白藜芦醇很少见,大多以葡萄多酚含量为主,并且与标准化的虎杖提取物复方成制剂。这种虎杖与葡萄提取物混合的知名商标成分包括:ResVinol®,ResveraTrue™,SuperPure®等
13.2.合成法生产:利用化学、生物和基因工程等综合方法制造白藜芦醇。
- 发酵法:例如意大利厂商通过真菌发酵获得的Veri-te™ 98%反式白藜芦醇原料。
13.3.提高生物利用度的专利成分:
总体而言,提高白藜芦醇吸收和生物利用度技术上仍没有明显的突破,这限制了其临床疗效作用。目前,市场上主要有以下二种提高白藜芦醇生物利用度的专利成分:
13.3.1.白藜芦醇水凝胶配方(Resverafen®):
将白藜芦醇与胡芦巴籽中的半乳甘露聚糖(Galactomannan)纤维结合,形成一种水凝胶涂层(Hydrogel coating),可以提高白藜芦醇的生物利用度。根据2022年发表的研究,这使血浆浓度(ng/mL)比未模拟的白藜芦醇高出约10倍222。
13.3.2. MicroActive®白藜芦醇:
将白藜芦醇微粉化,使其颗粒已经缩小到小颗粒尺寸(10μm),以通过静脉和毛细血管进行更大的传输。此外,MicroActive®白藜芦醇中聚合物的特殊混合物减缓了白藜芦醇的释放速度,因此它在血液中停留的时间更长。据未公开发表的文献,该技术使白藜芦醇生物利用度提高2.5倍(来源:MicroActive® Resveratrol官网)。
13.4.白藜芦醇产品类型和剂量:

综上所述,市场上白藜芦醇补充剂产品类型主要分为三类:
- 由标准化的虎杖提取物(标明白藜芦醇含量)与葡萄提取物(葡萄酒、葡萄皮等)复合的制剂。
- 仅由标准化的虎杖提取物配制,具有明确的白藜芦醇剂量。
- 其他:生物合成(如发酵)等。
大多数产品含白藜芦醇为:100mg,125mg,150mg,200mg,250mg,500mg等。剂型主要以胶囊形式为主,少量的片剂、液态制剂等。
